THE RULES

Ladies and Gentlemen, we inform you that:

1. services can be booked in person at the Angel Clinic reception desk, by phone, via e-mail at: info@angelclinic.pl or via the online booking portal on the website;
2. during registration, the staff will ask you for the following data: name and surname, telephone number and/or e-mail address. Please be advised that the registration procedure takes place with respect for your privacy;
3. refusal to provide personal data may result in refusal to provide the service;
4. when making a reservation, the Angel Clinic may request a prepayment in the amount of min. 20% of the price of the booked visit in accordance with the price list available on the website angelclinic.pl This amount will be deducted from the final amount for the service provided;
5. the fee referred to in point 4 of the Regulations may be made by traditional transfer to the account of Angel Clinic Sp. z o.o., in person at the clinic or via the online booking portal. Detailed information on the payment will be sent by e-mail if it is necessary to pay by traditional transfer;
6. the payment referred to in point 4 of the Regulations must be made no later than 1 hours before the visit in the case of booking via an online booking portal;
7. a consultation or visit can be canceled or postponed without incurring any consequences, including financial ones, no later than 24 hours before the date on which the visit was scheduled. Visits not canceled or canceled on the same day as part of purchased packages will be considered completed;
8. in the event of cancellation or postponement of the visit later than specified in point 7 of the Regulations, the prepayment referred to in point 4 will not be returned. It will also not be credited towards payment for the next visit;
9. for the first visit, please report to the clinic 10 minutes earlier. For the next visit you should arrive 5 minutes before it starts;
10. if the patient is late, the visit will not be extended. The time of delay is deducted from the time scheduled for the procedure. In justified cases, i.e. too much delay, the cosmetologist/doctor/dietician has the right to refuse to perform the service;
11. payment for the treatment is made after the service is performed at the Angel Clinic reception desk (cosmetology treatments) and directly at the doctor’s/dietician’s office;
12. the price for the services has been determined for individual parts of the body based on the surface of the body subjected to the treatment. The price for the treatment may change if the procedure is performed on a larger area of the body or if the preparation is used above the standard. The price list may change in the future;
13. laser hair removal treatment packages are valid for a year from the date of purchase, gift vouchers and other treatment packages are valid for 3 months from the date of purchase;
14. The patient has the right to access their dental and medical records.
